Senior Living Placement
Finding care outside the home for seniors and elders can be a very stressful situation. At Harbor Point Care our Nurse Liaisons can help you and your family find the higher level of care they need and desire. Whether you or your loved one needs; Nursing Home Care, Skilled Nursing, Assisted Living or an Adult Family Home we can help navigate the facility that best meets your needs. Placement services are free to the client, so fill out the form below and a Nurse Liaison will contact you right away. We typically select 2-3 homes that meet your criteria and you can chose to tour on your own or one of our RNs will assist you.
HPC has a growing network of Adult Family Homes, each home is visited and assessed on an ongoing basis. We work with AFH owners directly in Pierce and Kitsap counties. HPC takes pride in only selecting the homes that meet our criteria. We look for acuity of care provided, staffing to patient ratios, managers on site, cleanliness, any specialties, prior patient experience, how long the AFH has been around, pricing, activities offered, pets and grounds.
As an RN and owner of Harbor Point care, my primary objective is to assist families in finding placements that will last. I recognize the hardships caused by moving at this time of life and understand the necessity of finding situations that meet as many of my clients’ needs as possible.
