This is our most popular service we provide. An RN care manager or Master of Social Work will help you and your family develop a customized plan of care that will focus on the holistic care of nursing to reduce hospital readmission and cycling in and out of rehab. Our staff will help long-distance family members through decision making and will be the local point of contact. We will help manage medications and discharge orders at home and make sure the plan of care is followed. A free in home assessment will  be conducted to help increase safety in the home and prevent falls. Our RNs will help mediate family dynamics so everyone is on the same page with care while remaining informed and help make decisions. Our Care managers are trained to educate family on disease processes, and act as the liaison in choosing care in home or beyond the home. The care manager will work to develop a senior plan of care that will be updated as needs change. This service is customized to each individual family.
